Your Clinic Appointment

If you are scheduled to see one of the physicians or nurse practitioner for a new patient visit or consultation, you will need to go our office at 3355 RiverBend Drive, Suite 500, Springfield, Oregon, in the Northwest Specialty Clinics Building. (Please see the menu entitled, “Directions” for instructions on how to get to our office). In order for us to serve you in the best possible manner we request that you bring the following to your first office visit:

1. any requested forms or paperwork from our office
2. any pertinent medical records (at least 2-3 days prior to your appointment please check with your referring/primary care physician’s office to make sure that these records are sent to us at our FAX number: 541 685-5920)
3. a current list of your medications and their doses
4. your most current insurance information/insurance card

We also ask that you allow enough time to arrive early as unexpected delays do occur (trying to find our office, trying to find a parking space, etc.) We will try to accommodate every patient to the best of our ability; however, if you are late for your appointment we may have to reschedule you to another day.
